Break indices of Level 1 (BI 1) mark boundaries between prosodic words (PrWords). Items separated by BI 1 should carry one pitch accent each at most.

Demana-ho a la Maria!

The clitic pronoun ho cannot bear any accent on its own and has to be combined with the verb demana to form a PrWord. So demana-ho bears only one pitch accent and constitutes a PrWord.
Similarly, neither the preposition a nor the definite article la can bear a pitch accent on their own, they need to be combined to the name Maria to form a PrWord. So a la Maria bears only one pitch accent and constitutes a PrWord.
Thus, in this sentence, there are two pitch accents, each one aligned with the stressed syllable of the two PrWords demana-ho and a la Maria, and these two PrWords are separated by a BI 1.
